Dementia Link Workers

Our Dementia Link Worker service

Our Dementia Link Workers (DLWs) work alongside the GP Networks in Ealing and the West London Health Trust.

They support people (with and without dementia medication) who have been transferred from the cognitive and dementia services with a diagnosis of dementia.

The service is free and consists of the following:

Dementia Link Workers provide support and advice to people with dementia, their carers and their families, which often enables them to stay in their own homes.

This support includes regular assessments, providing information and appropriate referrals to a number of other services. Visit our Dementia Link Worker service on the NHS.

  • Assessment

    If dementia has been diagnosed, link workers will continue to assess the person with dementia and their carer.

    Our link workers are trained to complete these assessments in line with NHS standards. This assessment enables access to our services and those provided by other organisations.

  • Client Representation

    Our staff act as representatives of the carer and the person with dementia at meetings and to access services. We also link in and liaise with a range of organisations and professionals to coordinate care and other services.

  • Advice and Information

    We advise and inform carers about all aspects of dementia, how to care more effectively for a loved one with dementia, and explain all of the services available.

    This specialist advice also covers power of attorney, court of protection and welfare benefits. Additionally, we provide clients with the tools to navigate the social care system to empower each individual.

    If they're interested, we'll refer them to the other services offered by the charity, such as community support workers or social clubs.

  • Monitoring and Support

    We provide ongoing monitoring calls and a monitoring service for carers who initially feel they don't need any specific help, so we can support them if their situation changes.

    We support carers on an ongoing basis to help them come to terms with the diagnosis of dementia and relieve stress. With support, carers are more able to understand their role and make appropriate decisions in relation to caring.
